Family Care Coordinator (Licensed QMHP)

About Us:

ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. 

Primary Duties:

The Family Care Coordinator would be responsible for the service coordination for this project to ensure that there is designated liaison between all the PHC agencies(Hopewell, PHC, PCAC). The Coordinator will be the point of contact for referrals as well as supervision of the outreach CM. Position will be the initial assessment of need, offer wraparound services and referral to project. Project will work with pre-school and high school youth that have experienced trauma related to violence and other situations.

Education:  

A Master's degree in Human Service or a related field and at least one year of or behavioral health experience in a behavioral health setting. Applicants must be an eligible Qualified Mental Health Professional and Licensed Professional Counselor or Licensed Clinical Social Worker. Trauma experience preferred.
