JOB DESCRIPTION

This position is with the trauma team - the ages of the children you will be working with is 5-12 years.

ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. Hopewell Center operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth. Many services are provided at schools. Emergency services, including a crisis bed and mobile outreach, are available twenty-four hours per day. A multi-disciplinary team of psychiatrists, psychologists, nurses, counselors, social workers, educators, and support staff work diligently to carry out the mission of providing comprehensive care to patients and their families.

Duties and Responsibilities:
Hopewell Center currently has opportunities available for Community Support Specialist (CSS) in our Children Services Department (6-18 years). CSS's are responsible for providing community resource coordination and interactions that enable an individual at clear risk of hospitalization with School-based support/family support services. The position also involves transporting clients to appointments, and making school and home visits. The CSS also works with individuals who have been discharged from inpatient treatment to maintain an adjustment to living in the community.

The CSS is responsible for evaluation of client's status including personal strengths, areas of concern and needs. CSS worker will participate in formal assessment and the development of the person-centered plan; monitor client's progress towards goal attainment; provide assistance and act as a 24-hour resource when necessary. Train clients in living skills, social skills and other pro-social activities which enable client (s) to actively participate in the recovery process. The CSS worker is an active member of the Clinical Treatment Team. CSS worker is responsible for maintaining client records and assuring standards and guidelines are upheld.

Requirements:

Schedule 8am-5pm M-F
-Bachelor degree in Social Work, Psychology or other behavioral health sciences related field.

or

-Associates Degree and 2 years of experience in Community Mental Health, Case Management, Intensive In-Home services or Social work.

or

-4 years years of experience in Community Mental Health, Case Management, Intensive In-Home services or Social work.

-Must have valid Class E driver’s license, dependable transportation and personal vehicle insurance
