Training Bonus:  A $500.00 training bonus is paid after the completion of orientation, all assigned modules and 50 locked visits.

Job Description: The Home Health clinician (OT or ST) is responsible for providing direct care, patient education and the management of the applicable therapy (physical, occupational or speech) called for in the patients treatment plan. Consults with and educates other care team members concerning applicable patient assessment and treatment protocols. Serves as a rehabilitation resource for the care team. The care is delivered at the client’s home. This is a PRN position.

Education: Graduate of an accredited School of Physical Therapy/ Occupational Therapy/Speech Pathology with a Baccalaureate or Master s Degree.

Licensure and Certifications: Must have a current licensure by the Georgia State Board of the specified therapy and a valid CPR certification. Must possess a current valid Driver’s license.

Knowledge and Training: A professional knowledge of the theories, principles, practices and techniques inherent to the scope of practice. Communication skills to interact effectively with patient, family, care team members, referrals and community resources.

Knowledge of accrediting bodies standards, Medicare, Medicaid and other funding source regulations as applied to home care services. Demonstrates competent techniques and practices of their profession.

Computer Skills: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
