Job Description:

The Intake Coordinator interacts with discharge planners, case managers, clients and families by assisting with the initiation of home healthcare services and the discharge planning process. The Intake Coordinator would be responsible following the intake process at the office, and in some cases, may travel to various facilities  to directly intake the patient.

Job Responsibilities:

• Coordinate and arrange home care services between MedSide and facilities for home health

• services.

• Contribute patient care plan as it relates to the discharge plan.

• Provide information to ensure a smooth transition for patients and their families following hospitalization.

• Act as a clinical resource and educates hospital staff regarding the services offered by MedSide.

• Represent all divisions of MedSide Healthcare at various facilities

• In office position with flexible scheduling

Job Qualifications:

• Current Georgia LPN license with no sanctions.

• Excellent communication and interpersonal skills.

• Excellent attention to detail and organizational skills

• Extensive customer service experience (2+ years preferred)
