The Opportunity:

We are seeking a compassionate and dedicated Full-Time Licensed Practical Nurse (LPN) to join our home health team in Dekalb County. In this vital role, you will provide high-quality nursing care to patients in their homes, support their healthcare needs, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. This position requires excellent clinical skills, effective communication, and a genuine passion for helping others.

Job Responsibilities:

  • Provide nursing care to patients in their homes, including administration of medications, wound care, and monitoring vital signs.
  • Develop and implement individualized care plans in collaboration with patients and healthcare providers.
  • Educate patients and their families on managing their health conditions and maintaining a healthy lifestyle.
  • Document patient care activities accurately and timely in accordance with company policies and regulations.
  • Participate in regular team meetings and contribute to quality improvement initiatives.

Desired Qualifications:

  • Active LPN license in the state of Georgia.
  • Minimum of 1 year of nursing experience, preferably in home health or community health settings.
  • Strong clinical assessment and critical thinking skills.
  • Excellent interpersonal and communication skills to interact effectively with patients, families, and team members.
  • Ability to work independently and manage time efficiently.

Nice-to-Haves:

  • Experience with electronic health records (EHR) systems.
  • Certification in CPR and First Aid.
  • Knowledge of chronic disease management.
  • Additional certifications in specialized nursing fields (e.g., wound care, IV therapy).
