Our team focuses on providing a variety of services, including physical, occupational, and speech therapies, to individuals in the comfort of their own homes.

The Opportunity:

Health Watch Home Health is looking to hire an experienced and compassionate Physical Therapist (PRN, or Contract) to join our team and provide quality physical therapy services to our clients in their homes.

Job Responsibilities:

  • Complete and document initial physical assessments, after-care assessments, and patient progress notes in EMR system
  • Develop a personalized treatment plan for each patient
  • Administer and teach physical therapy interventions, exercises, and techniques to improve patient's mobility levels and functional abilities
  • Educate patients on how to maintain, strengthen, and increase their range of motion through exercises
  • Provide physical therapy services in accordance with established standards of practice and in compliance with all applicable regulations

Desired Qualifications:

  • A valid state license in Physical Therapy, or working towards such
  • Previous experience in physical therapy, home health environment preferred
  • Strong knowledge and understanding of strategies to help improve patient mobility, range of motion, and quality of life
  • Excellent communication skills and ability to build relationships with patients
  • A strong commitment to customer service and patient satisfaction

  • Experience working with electronic medical record systems
  • Reliability, good attendance and strong work ethic
  • Ability to self-manage workload and remain organized
  • Positive attitude and willingness to work as a team member
  • Excellent problem-solving skills and aptitude for, diagnoses, and treatments
