Job Description

                        CENTRAL SERVICES COORDINATOR

 TITLE:                        Central Services Coordinator

REPORTS TO:           Vice President of Sales and Marketing


  1. Has a high school diploma.

  2. Works as a team member and accepts verbal and written instructions.

  3. Has excellent observation and communication skills.

  4. Excellent telephone etiquette and professional demeanor.

  5. Is flexible and cooperative in fulfilling all obligations.

  6. Has working knowledge and skills with computer and all office machines.

  7. Is self-directed with the ability to work with little supervision.



Work closely with Mays sister agencies, patients/clients and co-workers for the smooth transition of referrals to the appropriate Mays agency.  Keep spreadsheets to easily access data for Administration’s review.   


  • Collect and organize all admissions, discharges and upcoming discharges on a daily basis from all agencies.

  • Review with plus all “on hold” clients for possible admission to Hospice or Home Health.

  • Sort all admissions into appropriate call lists and to manage prioritization and dissemination of client needs.

  • Review all discharge patients/clients for determination of possible referral to sister agency (necessity of services, dual eligibility, private pay).

  • Contact all patients/clients to make sure that the client was satisfied with services upon admission or discharge and to make sure they are going to call Mays again in the event they need future services.

  • Contact all referrals from sister-to-sister agencies and to update referring branch of status (unable to admit, client refusal, not qualified for services, home health, hospice, admitted to referred agency).

  • Contact referring branch to determine immediate need and to update the branch weekly on the status of the referral until admission.

  • Make follow-up calls to all clients on referral lists.

  • Develop spreadsheet to track status of clients referred and to manage information daily for quick reference and for future tracking (using graphs).

  • Report numbers of clients admitted as a result of central referral coordination department.

  • Create and manage spreadsheets and update daily.

  • Develop strong relationships with all branches.

  • Keep spreadsheet of referring person for bonus payouts.

General office with prolonged periods of computer usage



1. Visual/hearing ability sufficient to comprehend written/verbal communication.

2. Ability to perform tasks involving physical activity, which may include prolonged sitting, lifting less than 35 pounds and extensive bending and standing.

3. Ability to deal effectively with stress and frequent interruptions. Requires high level of flexibility.
