OUTREACH COUNSELOR (CAC-AD)

The primary responsibility of the Identities Outreach Counselor is to provide Substance abuse related outreach for youth participating in the Identities Program.
*Provide bi-weekly individual counseling to each client in assigned caseload.
*Facilitate group substance abuse education sessions with all assigned clients once a week.
*Facilitate family counseling sessions with each assigned client twice a month, or more often if necessary.
*Complete and update Individual treatment plans for each assigned client on a monthly basis.
*Maintain appropriate written and verbal communication with the referral agency for each assigned client.
*Enter appropriate documentation in client records on a daily basis.
*Participate in weekly treatment planning meetings and multi-disciplinary meetings.
*Develop and facilitate a substance abuse prevention and education curriculum.
*Facilitate weekly relapse prevention group meetings.
*Explore resources for clients living with addiction (i.e., community NA/AA meetings) and make referrals as necessary.
Minimum Education Requirements
*CAC-AD, LPC, LMSW
*Experience as a licensed /certified counselor
