We are currently partnering with a local primary care clinic to find the right candidate for the following position:
Home Visiting Nurse Practitioner
Responsibilities: Manage the transitional and chronic care needs of homebound patients including:
- Assessment and Evaluation of current condition
- Take past medical history
- Korean Speaking
- Review symptoms
- Conduct general physical examinations
- Review and reconcile medications
- Screen for depression
- Screen for alcohol and drug abuse
- Identify diagnoses to be used in care management and active medical management for purposes of treatment
- Recommend appropriate measures, including medication prescriptions and skilled needs for the patient
- Efficiently communicate findings in your assessment to the office / supervising physician
- Educate patients on topics such as disease process and prevention, medication, and compliance
Qualifications:
- Current NP license, with no sanctions, restrictions, or provisions, in the state of Georgia
- Russian Speaking
- At least 1 year of clinical experience
- Clinical background in adult, family or geriatric specialties
- Experience working with an Electronic health record (EHR)
- Effective communication skills are critical
- Ease of use with smartphone and email are required
- Reliable transportation
- Ability to work independently
- Home care or home visiting experience (preferred)
- Willingness to spend at least 30 minutes to 1.5 hours with a patient in their homes
