FULL TIME LICENSED PRACTICAL NURSE (LPN)

for the following counties:

• DEKALB

Job Description:

• Perform skilled nursing services including: wound care, lab work, patient education, etc. under a physician’s order

• Report any changes in the patient’s physical condition and health care needs to the supervisor

• Assist patients, as needed, with medications; observe for possible reactions

• Complete an in-home supervisory visit with assigned patients at least once per year

• Prepare progress notes and supervisory visit entries, documenting patient's current status

• Collaborate with the supervising RN to update patient care plans

• Conduct annual Personal Support Aide (PSA) performance evaluation

Job Qualifications:

• Must be willing to travel to and work in patient homes

• Graduate of an approved Professional Nursing Program.

• Current LPN license, with no sanctions, restrictions, or provisions, in the state of Georgia - or - applicable LPN Compact license

• One year of related experience in a clinical environment
