Stability Housing Case Manager

Direct Services | Pontiac, MI | Full Time

Job Description

Lighthouse is seeking a full-time Stability Housing Case Manager to assist formerly homeless individuals and families in securing and maintaining safe and affordable housing, and increasing their overall health and stability. This position will spend much of its time in the field conducting home visits, and will be stationed at our Pontiac office with office space at our Lathrup Village office.

ABOUT US:

With the mission of building communities that end poverty, Lighthouse works to meet the needs of those facing economic crises, including hunger and homelessness, throughout Oakland County. Lighthouse offers a wide variety of services to assist people experiencing homelessness and poverty, including: a rotating emergency shelter, food pantries, case management, and various housing assistance programs.

Responsibilities:

  • Conduct screenings, intakes, exits, and SPDAT assessments using the Homelessness Management Information System (HMIS)

  • Maintain a caseload of at least 20 households participating in Lighthouse’s Permanent Supportive Housing (PSH) programs,

  • Provide intensive in-home case management services with at least monthly contact,

  • Provide clients with ongoing information and referrals including coordinating services between providers,

  • Enter data timely within HMIS, and complete live case notes when possible,

  • Track case management activities and client outcomes in HMIS as well as in an internal tracking tool,

  • Perform asset/need assessments and develop Individual Service Plans to map out goals,

  • Using the Mainstream benefit Access Tool, assist clients in applying for and obtaining mainstream benefits including, section 8, Homeless Preference Voucher program, MDHHS benefits, Social Security, and Earned Income Tax Credits, etc.

  • Foster inter-agency collaborative solutions to help clients attain long-term housing and  financial stability,

  • Ensure the proper maintenance of client files and records,

  • Provide time-sensitive activity reports, and collect data to support the measurement of program outcomes,

  • Assist with front-desk operations, answering phones, and crisis calls as needed.

Outcomes/Outputs:

  • Assist homeless individuals and families with an expedient “homeless to housing” process: less than 35 days (intake to lease signing)

  • Assist households in graduating PSH into less intensive housing programs,

  • Aid in increasing levels of stability – measure increases via SPDAT,

  • Ensure 100% of clients interested in enrolling in follow-up care are referred – smoothly transition cases.

Requirements:

  • Bachelor's degree and 1-2 years of experience in human services preferred,

  • Experience working with low-income individuals and families,

  • Proficient with Microsoft Office Suite and Google Apps,

  • Strong organizational and communication skills,

  • Strong conflict resolution skills,

  • An appetite for innovation and creative problem-solving,

  • Ability to work evenings and some weekends,

  • Reliable transportation.

Preferred:

  • Social Work degree,
  • Professional or lived experience with homeless populations,
  • Some knowledge of the social services landscape in Oakland County and surrounding areas, 
  • Experience with Homeless Management Information System.
