Population Health Manager

Job Description

Summary of Position

Under the direct supervision of the Director of Population Health and Case Management , this individual will work collaboratively with the panel managers, Quality Department , physicians, advanced care practitioners, Informatics and Information Technology Department, staff and other health care professionals to provide Value Based Care for patients and providers. This position will focus on improving patient care, quality and financial savings through care management initiatives. This position will have primary oversight and management of the panel manager team, whose focus is on outreach and communication with patients for coordination of healthcare needs, including gaps in care, identification of risks, social determinations of health, transition of care management, annual wellness visits, patient attribution and linkage to community resources. This position will work closely with the lead panel manager to keep the the panel manger team engaged, align efforts of our Clinically Integrated Networks initiatives, maintain consistency in workflow across the organization and use data analytics to drive team and clinic improvement. This position will also work closely with the Director of Population Health and Case Management to manage value based programs (i.e. Medicaid Managed Care, Medicare and Commercial ACOs).


Minimum Qualifications and required Skills

  • A minimum of 3 years of related work experience in case management

  • Management and Leadership Experience

  • Understanding of quality and healthcare initiatives

  • Understanding of Case Management Requirements for Managed Care Contracts

  • Proficient in use of EMR’s (Electronic Medical Records) and Microsoft Word Suite

  • Excellent communication skills (written and verbal)  


Education: Registered Nurse, unrestricted license in the state of North Carolina

Certification: BLS/ Case Management Certification   

Key Responsibilities 

  1. To monitor and oversee the performance and productivity of employees within the department. 

    1. Managing leave requests, time logs and expense reports:

  2. Handling employee/personnel issues in accordance with company policy; consulting with the Director and others as needed.

    1. Keeping the  Population Health Case Management Program Director fully informed and apprised of department accomplishments and issues or problem areas;

  3. Assuring adequate coverage of the department;

  4. Focusing on meeting or exceeding department performance and productivity goals; and seeking ways to improve performance and efficiency of the department.

  5. To ensure the programs meets the contractual or grant requirements or payor requirements.

  6. To develop effective working relationships with colleagues and practice management staff.

  7. Demonstrates attention to detail in the design, development and implementation of programs or projects.

  8. To follow-through on commitments to contractors, grantors or payors.

  9. To use good judgment and seek advice/guidance from the Case Management Director frequently.

  10. To develop reports and communicate findings and areas of concerns with the Case Management Director and Chief Quality Officer.

  11. To meet target dates for work and produce quality work.

  12. To demonstrates organizational skills and time management skills.

  13. Perform other duties as assigned
