Licensed Practical Nurse (LPN)

Summary of Position: An LPN must perform nursing care essential to the assessment, promotion, maintenance and restoration of patient’s health and wellness in the assigned setting. The LPN performs a variety of duties related to patient care, therapeutic interventions and clinic-based supportive functions as directed by an MD, mid-level provider, RN and or the assigned supervisor. The LPN must be able to support patients to utilize and benefit from the clinical services, perform triage functions with minimal supervision, and execute activities consistently within their state required LPN scope of practice. The LPN may be assigned to support the scope of services provided at Kintegra Health including but not limited to family practice, pediatrics and geriatrics. LPNs may be asked to cover a different Kintegra Health location from their assigned clinic.

• Ability to plan, prioritize and complete delegated task with attention to detail

• Ability to mentor medical assistant staff

• Ability to work well under pressure and with time constraints

• Ability to communicate and work effectively with the physical and emotional development of all age groups

• Ability to learn and retain information regarding patient care procedures and their documentation

• Familiarity with basic computer operations, MS Word- Excel and use of  e-mail communication

• Ability to work successfully with indirect supervision and autonomy

• Neat and well-groomed appearance

Education: Graduation from an accredited LPN program

Experience: 3-5  years of related work experience preferred

Professional Licensure: Current and unrestricted North Carolina LPN licensure

Certification(s): CPR

Key Responsibilities:

• Able to independently execute care responsibilities in a manner that facilitates efficiency workflow. 

• Assist patient in navigating care through the encounter process to minimize ambiguity  

• Accurately document all aspects of patient encounters required for job functions

• Tracks and document closed loop communication with patients, including but not limited to labs, return calls, refills and more. 

• Communicate with external and internal parties as needed to facilitate continuity of care. 

• Fulfills supportive assignment on time and unprompted. Assignments may include but are not limited performing controls, instrument maintenance, infection control, ordering, sorting, storing supplies, and restocking

• Patient teaching and care plan support 

• Guard patient privacy and comply with HIPAA regulations at all times

• Actively participates in QI activities.

• Participates in staff meetings as required 

• Complete annual competencies within the required time frame
