Lead Team Coordinator

Senior TLC PACE Program | Gastonia, NC | Full Time

Job Description

Job Summary: Under the direct supervision of the Director of Quality and Corporate Compliance, the Lead Team Coordinator is responsible for approving time for the Team Coordinator Team. The Lead will be responsible for ensuring all authorizations for services approved by the IDT are timely and correct. The Lead will be responsible for their team by coordinating and scheduling all internal and external appointments for Senior TLC participants. Works with all STLC medical and transportation staff, as well as outside contracted services. The Team Coordinators will maintain responsibility for the maintenance, security, and distribution of medical records for the Senior TLC participants and prospective participants. The Team Coordinator will maintain documentation of assigned team’s interdisciplinary team (IDT) minutes and Care Planning.

Education: High School Diploma

Experience: Minimum one year of employment in a medical office setting, or equivalent business office experience. One year of experience as a Team Coordinator preferred.

Licensure, Registry or Certification Required: NC driver’s license

Special Training: Must be able to work effectively in a team environment. Must possess excellent writing, reporting, communication, and interpersonal skills. Proficient in Excel and Word.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Ages of Patients Rendered Care: Adult and Geriatric

Key Responsibilities:

  • Supports team members and functions as a team player while providing that support and adheres to the “do no harm” teamwork statement.

  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.

  • Ensures the physical and electronic security of all records in accordance with acceptable standards of practice.

  • Ensures timely and accurate entry of authorizations and will be the lead for any questions regarding authorizations of services by the IDT.

  • Coordinates routine group rounding times for team coordinators as determined by Lead Team Coordinator & Director of Quality and Corporate Compliance and assists with approving time off for the team coordinators

  • Communicates with other care providers when providing or collecting participant medical information.

  • Assembles medical records of assigned team’s participants for team use. Provides for the quick and accurate retrieval of information as need by the interdisciplinary team (IDT).

  • Assists Health Plan Operations as requested.

  • Maintains current directory listing such as essential telephone numbers. Maintains calendar of appointment dates and times of participants.

  • Coordinates and schedules all medical related appointments for assigned team’s participants and STLC physicians both internal and external, communicating with doctors, nurses, and other medical staff.

  • Coordinates and schedules all imaging for participants. Calls and pre-registers participants for appointments.

  • Coordinates and communicates with the transportation department, home care coordinators, and center nursing for any participant needs, appointment times, location, and any changes for participant outside appointments.

  • Communicates appointment times and location to family members as needed.

  • Attends and records team’s IDT meetings and Care Planning sessions.

  • Other duties as assigned.
