Diabetes Navigator

Job Description

Summary of Position

The Diabetes Navigator will support the Diabetes Care and Education Specialist by addressing diabetic patients’ barriers to health related to social determinate. The Diabetic Navigator will assist in coordination of internal referrals for diabetic patients and work to connect the patient with community resources to meet needs impacting care such as, transportation, food, housing, employment, and exercise. The Diabetic Navigator will assist patients in enrollment to Super SNAP, Medicare/Medicaid, ACA health insurance plans, and make needed referrals for other communal/support needs of the patient. The Diabetic Navigator must remain flexible and possess exceptional communication skills.

Education: High School Diploma or equivalent and RMA/CMA. Licensed Practical Nurse or Social Worker Preferred 

Minimal Certification: Basic Life Support Training and Mental Health First Aid (may obtain within six months of employment.

Required Experience: Preferred Care Coordination Experience 2+ years

 Key Responsibilities

  • Coordination of internal referrals for diabetic patients identified by Diabetes Care & Education Specialist.

  • Assistance with medication approval/coverage by insurers

  • Support patients in ACA, Medicare/Medicaid enrollment or make proper referral

  • Seek and provide resources to meet needs impacting care

  • Accompany the Diabetes Care & Education Specialist in group classes and clinic days, if needed

  • Participate and/or coordinate Community Outreach for Diabetic Prevention and health promotion such as health fairs and other community events.

  • Assist in continuity of care by following-up with diabetic patients referred for services

  • Attend ENDO clinic to provide social determinate needs assessment on all diabetic patients in clinic and linkage to communal/internal resources per provider and/or based upon assessment

  • Coordinate internal diabetic patient referrals working with Triage Nursing Staff to ensure patients receive timely care

  • Collaborate with other communal entities to identify common health disparities and needed education

  • Perform other assigned duties
