Job Duties/Responsibilities/Functions (including but not limited to)
• Provide care for outpatients as needed.
• Perform chart reviews and attend medical staff meetings as part of the normal workday.
• Treat those requiring musculoskeletal care on any given duty day.
• Interview patients, diagnose injury, prescribe treatments and refer patients to military or civilian hospitals for more complex musculoskeletal problems.
• Perform only those services for which privileges have been granted by the credentials committee.
• Complete medical records, forms and documents within time limits specified.
• Check e-mail daily since e-mail is the official method of internal communications, x rays, referrals,
• etc. via the CHCS computer system.
• Request consultations and procedures as considered appropriate.
• Study health records and evaluate the significance of total clinical evidence.
• Make periodic follow up examinations of patients to determine progress and reaction to treatment, review clinical evidence to detect deviation from normal disease course, and determine the patient's progress. When deemed appropriate, shall consult with other physicians concerning the significance of symptoms and interpretations, prepare clinical reports of each examination, consultation and diagnosis and record other pertinent data in the appropriate format.
• Maintain accurate and up-to-date records, including daily man hours.
• Utilize the electronic medical record system for keeping records, ordering of ancillary procedures, tracking of medications, writing orders and performing other required patient record functions.
• As workload permits during scheduled duty, contract providers shall attend meetings as required.
• Become familiar with the hospital formulary.

• Medical records completed shall be subject to review by a medical records review committee and shall be notified of deficiencies in his/her entries and shall be instructed to correct the deficiencies.
• Utilize standardized, template patient encounter sheets as supplements in order to facilitate documentation of patient care.
• Verify the content and correctness of all prepared and transcribed reports by affixing his/her own signature and stamp to all copies of the document and validating its contents.
• Document appropriate history, changes, care plan, examinations and diagnosis (as appropriate) prior to and during healthcare treatment.

Minimum Qualifications
• Graduate from college/university, accredited by the American Physical Therapy Association (APTA), with a degree in Physical Therapy.
• Possess and maintain a current unrestricted license to practice as a Physical Therapist.
• Certified/maintain current Basic Life Support (BLS - certification shall be from the American Red Cross (ARC).
• Ability to type and have experience using computers, Microsoft programs especially.
