Medical Records Coder-Senior
Work Location: San Antonio, TX

Job Summary/Description
The Medical Records Coder will use medical codes to document healthcare diagnosis, procedures, medical services, and equipment into universal medical alphanumeric codes. The diagnoses and procedure codes are taken from medical record documentation, such as transcription of physician's notes, laboratory and radiologic results, etc. Medical coding professionals help ensure the codes are applied correctly during the medical billing process, which includes abstracting the information from documentation, assigning the appropriate codes, and creating a claim to be paid by insurance carriers. The work schedule is Monday through Friday 8a-5p.

Job Duties/Responsibilities/Functions (including but not limited to)
• Conducts the quality review of inpatient and outpatient coding.
• Assures coding compliance with federal regulations.
• Maintains up-to-date coding guidelines and coding policy changes.
• Reviews, interprets and assigns diagnostic codes based on medical record documentation according to correct coding principals.
• Provides skilled and specialized technical work in documentation and coding for medical billing.
• Abstracts complex patient-related data from medical records and codding of diagnoses and procedures using ICD9/10 and CPT codes.
• Works coding related charge review and claim edits daily to ensure timely and accurate billing.
• Researches and resolves coding related issues and assists in meeting productivity and quality standards.
• Performs other duties as assigned.

Minimum Qualifications
• Requires registration, certification or licensure with the appropriate agency.
• High School diploma and five years’ experience in medical record abstraction and coding with accreditation from a professional coding organization (AHIMA, AAPC, RHIT, RHIA or CCS certification).
• Must be certified with AAPC or AHIMA.
• Prefer Emergency Department professional coding experience.
• Experience coding primarily E/M codes with some bed side procedures.
