Consultant - Certified Care/Case Manager (RN)
Work Location: Harrisburg, PA

InGenesis is currently seeking a Registered Nurse who is already or willing to become a Certified Care/Case Manager (CCM) to serve as a Consultant and perform the six (6) essential activities of case management: assessment, planning, implementation, coordination, monitoring, and evaluation.

Job Duties/Responsibilities/Functions (including but not limited to)
• Evaluate case data, including clinical information, medical history, level and location of care, functional status, psychosocial and environmental factors.
• Document assessment, goals, plan of care and timetables.
• Plan alternatives based on recipient needs and investigate opportunities to use available community resources.
• Utilize Medical Assistance regulations for prospective review of medically necessary services and procedures.
• Educate the recipient and/or family on the availability of community resources and services.
• Work closely with medical directors to implement appropriate action plans.
• Manage a caseload of 75-100 cases.
• Maintain management records in accordance with professionally accepted standards.
• Performs other duties as necessary or assigned.

Minimum Qualifications
• Experience: 2+ years’ experience in case management or related field.
• Licensure: Active RN license as issued by the state of Pennsylvania.
• Certifications: Certified Care/Case Manager certification required within 12 months of start date.
• Basic computer skills, required.
