Medicare Patient Educator/CMA

Job Description

The Medicare Patient Educator works closely with a Nurse Care Manager to support the operation of practices serving Medicare patients and implement activities that are associated with Medicare reimbursement for Annual Wellness Visits, Chronic Care Management and Transitional Care Management for Community Health Partners (CHP)

This Patient Educator will contact Medicare patients, make appointments, and provide a range of duties to help the Nurse Care Manager with the overall program efforts to support an Accountable Care Organization fulfilling the performance and quality requirements for reimbursement and shared Medicare savings. The Patient Educator works within the clinic/practice setting or remotely to help manage the practice’s Medicare patients, communicating and documenting services and interventions.

The Patient Educator is focused on improving patient care, better utilization of services and reduced medical costs. The position reports to CHP Chronic Care Program Manager and works directly with the practitioners, the medical office staff and the Nurse Care Manager. The position is supported financially by the practice group or clinic through a contract with Community Health Partners.

Minimum Qualifications for Position

  • Has four years of work experience in the health and is a Certified Medical Assistant (CMA) through the American Association of Medical Assistants (AAMA) or

  • Demonstrated ability to learn health matters and be educated on the basics of chronic diseases management

  • Self-Motivated

  • Ability to follow a variety of operational details and program requirements

  • Ability to work as part of a team to enhance the achieve patient health goals

  • Strong communication skills and demonstrates good judgement

  • Familiar with practice operations and work flows

  • Strong organizational, analytical and time management skills

  • Competence is establishing rapport with patients and providers

  • Ability to work independently as well as be a team player

  • Basic skills in use of computer software for patient management, Microsoft Office- Word, Excel

  • Bi-Lingual is preferred
