First To Serve, Inc. provides state certified and licensed substance abuse and supportive housing facilities located throughout the city of Los Angeles. First To Serve also provides Domestic Violence Housing for single women and women with children, and participates annually in the Los Angeles Homeless Services Authority supported Winter Shelter Program.

Job Title: Case Manager - S. San Pedro Street

Division/Department: (FTS) First to Serve

Essential Duties and Responsibilities: 

The Case Manager will provide case management services to FTS clients providing direct support in the attainment of self-efficacy; including the successful transition into and maintenance of permanent housing.

Additional responsibilities are but not limited to:

  • Provide an individual needs assessment, biopsychosocial assessment, and Treatment/Individual Service Plan(s)

  • Aid individuals and/or families requiring permanent housing, social service, mental health, medical, domestic violence and parenting assistance (as needed)

  • Locate and make use of appropriate community resources and linkage to services for clients;

  • Evaluate clients for readiness to transition including the formulation of service plans and goals, and aid clients to implement service plans

  • Provide individual attention to each client/resident in the form of weekly case management meetings to be on a one-to-one basis

  • Attend Multidisciplinary Case Planning and Supervision Meetings with supervisor as scheduled

  • Communicate effectively and weight the opinions and feelings of all clients and their family members

  • Work on special projects (as needed)

  • Maintain compliance with all company policies and procedures

  • Perform related duties as assigned by supervisor


  • Must possess a Bachelors Degree in a Social Services field and minimum one year of relevant experience or a combination of education and experience

  • Speak, write and read fluently in English. Bilingual a plus

  • Excellent verbal and written communication skills

  • Familiarity with community resources, agencies etc. that provide Permanent Housing, Mental Health, Domestic Violence and other services including but not limited to medical assistance

  • Ability to establish effective working relationship with staff, community agencies and clients

  • Ability to recognize and be sensitive to race, color, religion, sex, national origin, age, disability and  genetic information

  • Personal means of transportation preferred

  • Ability to safely perform essential job functions, with or without reasonable accommodations

  • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service to internal and external clients

  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
