About 1st Care Management:
Our mission at 1st Care Management is to offer a new approach to patient care by eliminating the burden of office visits for patients unable to do so. We promise care to our patients within the comfort of their own homes, on schedules that work best for them.
By utilizing Transitional Care Management and home visiting practitioners, our collective goal is to decrease the patient’s risk of hospitalization. We want to optimize their clinical outcomes through personalized and effective care. Our Chronic Care Management program offers patients a support system to navigate through chronic conditions just like they would in a primary care setup, now from their own homes. All of our services combined allow us to lessen our patient’s burden at a time when they need help the most.
