Job Description

Clinical Care Coordinator

Doctor On Demand has identified the need to establish positions that allow clinical staff who have demonstrated both training and competency in clinical care to aid in delivery of clinical services. These positions may be staffed by allied health professionals with certifications or documented licenses within the US as Registered Nurses or Registered Nurse Practitioners.

Clinical Care Coordinators will support Medical Directors and Clinical Leads, Doctor On Demand physicians, Patients, Customer Support Agents, Health Systems personnel and any other licensed professional aiding in or participating in the care of Doctor On Demand patients. A position description will follow, but specific services may include care coordination, safety and quality programs, patient assessment and education, chart auditing and patient education on general topics, care navigation and coordination with other care team members, data and program analytics, chronic disease management program implementation. Performance evaluation will include progress on individually self-identified goals, patient and program outcomes, 360 degree evaluation by colleagues, practice leadership, support staff and patient feedback/ratings.

Position Description:

Clinical Care Coordinator incumbent will work collaboratively with Medical Directors, Clinical Support and physician staff to enhance the clinical offerings of Doctor On Demand. The roles and responsibilities of this role will include, but are not be limited to the following:

Clinical Care -

- aid in care coordination, directing patients to referral completion to in-person care & navigating the health system

- performing follow-up functions, including contacting “flagged” patients to gather data around response to treatment or progression of illness,

- performing medication reconciliation and audit reported medications and allergies to ensure accuracy,

- educating patients on health topics

Clinical Quality (QA/QI) -

a. Working both collaboratively and individually on QA/QI initiatives

b. Reporting program outcomes to Medical Directors and other clinical leadership

c. Documenting intake and patient assessments, coaching sessions, phone visits and text messaging

Communication -

a. Providing and receiving feedback from clinical staff

b. Reporting ethical, professional and moral concerns to Quality Review Committee

c. Coordinating clinical care program operations with clinical, sales, marketing, product teams

d. Excellent listening skills, empathy

Requirements -

a. Minimum of 5 years RN clinical care and direct patient experience

b. Empathic person with strong active listening skills who can learn and execute motivational interviewing techniques

c. Background in program design and implementation

d. Relationship-centered patient care approach

e. Experience working on multidisciplinary care teams

f. Excellent time management and problem solving skills

g. Health coach and case management experience preferred

h. Strong presentation skills with ability to participate in sales calls and externally face clients
