Organization Overview
Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own and manage multi-family supportive housing and licensed residential care homes, and implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services and health care sectors.

Position Summary

The Social Worker will manage a case load of 30-35 homeless Veterans participating in the HUD-VASH program. The HUD-VASH program enables Veterans to secure housing by providing access to HUD Section 8 vouchers. The Social Worker will assist the Veteran in securing housing and will also be responsible for following their case load once the Veterans have been successfully housed to ensure housing retention. The Social Worker will also develop a treatment plan and manage to the objectives listed in the treatment plan.

Responsibilities

1. Providing outreach services to engage Veterans experiencing homelessness, especially Veterans who are in the following groups: chronically homeless, those with families, women, with disabilities, and those who served in the Operation Enduring Freedom (OEF), Operation Iraqi Freedom (OIF), and the Operation New Dawn (OND) eras.

2. Verifying the Veterans status, eligibility for VA medical care, and clinical need for program participation.

3. Screening and a brief assessment to ensure appropriateness of placement into the program.

4. Assessing Veterans through comprehensive psychosocial evaluations to determine case management needs and recovery goals.

5. Providing access to appropriate treatment and supportive case management services to potential HUD-VASH Program participants. NOTE: This can be done individually or in groups.

6. Employing Motivational Interviewing and Harm Reduction approaches to promote Veteran follow through with referral for preventive care and treatment of medical conditions, substance use and dependence, other mental health conditions, and problematic health behaviors (e.g., problematic substance use, tobacco use, unsafe sexual practices).

7. Helping the Veteran obtain the voucher subsidy from PHA.

8. Providing housing search assistance to HUD-VASH participants with rental vouchers.

9. Making regular home visits to assess Veterans housing stability, social connection and recovery, and acting as a liaison with other VA and community resources, landlords, and PHA. Regular re-assessment and revision of treatment plans is done as needed.

10. Ensuring coverage of caseload during absences.

11. Reviewing agreements between Section 8 HCV landlords, PHAs, and VA in establishing appropriate referral sites for HUD-VASH placements.

12. Performing site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.

13. Meeting regularly with landlords and PHA officials to ensure the availability of Section 8 HCV eligible housing stock.

14. Collecting and submitting HUD-VASH Program evaluation data on Veteran family participants.

15. Ensuring that there is no conflict of interest in dealings with Veterans, landlords, or other entities by adhering to professional ethical guidelines. It is essential that all staff complete annual ethics training, to include conflicts of interest, provided as indicated in subparagraph

16. Participating in program specific conference calls, broadcasts, and regularly scheduled meetings such as:

a. Monthly 1-hour conference call hosted by the HUD-VASH National Director. Program policy, trends, and resources, as well as site-specific issues and new program ideas are addressed in this forum. A reminder is sent with conference call information.

b. Conference calls led by the VISN NHC are also strongly encouraged to promote training, discuss local issues, share information, and develop a supportive mentoring resource.

c. Viewing of available HUD-VASH satellite broadcasts, PowerPoint presentations, and attendance at national and regional HUD-VASH and other homeless conferences are encouraged, as time and funding permits.

17. Must be willing to provide transportation for veterans to various appointments as needed.

Professional Experience

Master Degree in Social Work from an accredited school of social work;

A valid, clean CA driver’s license and a personal insured vehicle are required. Experience working with individuals with mental health, disabilities, substance abuse, or HIV/AIDS related issues;

Experience working in a crisis setting and ability to respond appropriately in an emergency;

You have strong background in case management and understand what it takes acquire and utilize Section 8 vouchers, prevent eviction and other housing related issues.

Knowledge, Skills and Ability

Knowledge of affordable housing related housing experience;

Knowledge of Fair Housing Laws and reasonable accommodations;

Excellent verbal and written communication skills;

Strong interpersonal skills and client engagement skills;

You recognize the impact of stable community-based supportive housing to one’s health and wellbeing.

Possession of a valid California driver’s license, a clean driving record, and automobile insurance;

Access to adequate transportation;

Basic computer knowledge, MS Word and Excel required;

PowerPoint preferred;

Flexibility required regarding scheduling and prioritizing of tasks;

Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment;

 Ability to work independently;
