A leading healthcare provider is seeking a Physical Therapist, Inpatient Rehabilitation, to evaluate and treat patients of all ages in an inpatient rehabilitation setting, develop individualized treatment plans, deliver evidence-based therapy services, educate patients and caregivers, and collaborate with interdisciplinary teams.
Physical Therapist, Inpatient Rehabilitation Responsibilities:
- Evaluate patients of all ages and develop individualized treatment plans that integrate functional goals, therapeutic interventions, expected outcomes, and discharge planning.
- Deliver evidence-based physical therapy services, monitor patient progress, and modify treatment plans as appropriate based on patient response and clinical findings.
- Perform safe patient transfers and mobility training with assistance as needed while utilizing proper body mechanics and promoting a safe environment for patients and staff.
- Educate patients, family members, caregivers, students, and healthcare professionals using effective teaching methods to support recovery, independence, and continuity of care.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care and facilitate successful rehabilitation outcomes.
- Serve as a clinical resource and professional role model by demonstrating compassionate, respectful, and patient-centered care while supporting staff and student development.
Physical Therapist, Inpatient Rehabilitation Qualifications:
- Bachelor's degree in Physical Therapy.
- Current Illinois Physical Therapist (PT) License.
- Basic Life Support (BLS) certification through the American Heart Association (AHA).
- Experience providing physical therapy services in an inpatient rehabilitation or acute care setting is preferred.
- Knowledge of evidence-based rehabilitation practices, patient assessment techniques, treatment planning, and discharge planning.
- Strong communication and interpersonal skills with the ability to educate patients, families, caregivers, and interdisciplinary team members.
- Ability to safely perform patient transfers and mobility assistance while demonstrating proper body mechanics and patient safety practices.
Shift:
- Monday to Friday, 8:00 am to 4:30 pm.
