A leading medical institution is seeking a Patient Navigator Specialist. The successful candidate will serve as a non-clinical resource and liaison for patients and their families by providing individualized support throughout the hospital stay and/or clinic visits, and developing expertise in the unit or clinic operations and systems to resolve issues across departments.

Patient Navigator Specialist Pay and Benefits:

• Hourly pay: $40/hr
• Worksite: San Francisco, CA 94143 - Hybrid.
• W2 Employment, Group Medical, Dental, Vision, Life, Retirement Savings Program, PSL
• 40 hours/week, 6 Month Assignment

Patient Navigator Specialist Responsibilities:

• Provide support during patient stay or clinic visit; facilitate scheduling, link patients to services across the health system.
  Provide new patients with an overview of resources and general information on billing, scheduling, and locating resources for clinical questions or concerns.
• Assist as liaison with other departments to facilitate improved coordination of care.
• Anticipate patient care coordination needs; provide a general road map to patients to set expectations.

Patient Navigator Specialist Qualifications:

• 1+ years of experience with new patient scheduling/coordination and medical records retrieval.
• Bachelor's degree in a related area.

Shift:

• Monday to Friday, 08:30 am - 05:00 pm.
