A leading healthcare provider is seeking an Acute Care Physical Therapist V to evaluate and treat patients of all ages in an acute care setting, develop and modify individualized plans of care, deliver high-quality physical therapy services, serve as a clinical resource for staff and students, and promote excellence in patient-centered rehabilitation.

Acute Care Physical Therapist V Responsibilities:

  • Evaluate patients of all ages and develop individualized physical therapy treatment plans based on clinical assessments, functional needs, and patient goals.
  • Provide evidence-based physical therapy interventions while modifying plans of care as appropriate to optimize patient outcomes and functional recovery.
  • Monitor patient progress, document treatment effectiveness, and adjust interventions to ensure safe, high-quality rehabilitation services.
  • Serve as a role model in the delivery of professional physical therapy services while acting as a clinical resource for staff and students.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams to coordinate patient care and achieve optimal rehabilitation outcomes.

Acute Care Physical Therapist V Qualifications:

  • Bachelor's degree in Physical Therapy.
  • Current Illinois Physical Therapist (PT) License.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Knowledge of physical therapy evaluation techniques, treatment planning, rehabilitation principles, and patient care.

Shift:

  • Monday to Friday, 8:30 am - 5:00 pm.
